Supporting safe, informed and successful transitions
Supporting children and young people through key points of transition is a central part of HEP’s work. Working collaboratively with schools, the Local Authority and partner organisations, HEP coordinates a range of initiatives that strengthen communication, improve safeguarding and ensure pupils receive the support they need as they move between phases of education.
- Our transition offer currently includes:
SixIntoSeven – a secure digital platform for sharing safeguarding, SEND and contextual information between primary and secondary schools. - DSL Handover Meeting – an annual borough-wide event bringing together safeguarding leads to support effective pupil transition through professional dialogue and information sharing.
- Primary to Secondary Transition – supporting the safe transfer of safeguarding, SEND and contextual information between primary and secondary schools.
- Secondary to Further Education Transition (new for 2026) – extending collaborative transition arrangements to support the effective handover of safeguarding information and continuity of support as young people move from secondary schools into further education settings.
